Report an adverse event

If it is an emergency, call 911. If you think an ORIGEN product has made you unwell, stop taking it and see a health professional.

• Tell us: with the form below or by writing to [EMAIL EVENTOS ADVERSOS — PENDIENTE]. Say which product, the lot if you can see it on the label, when you started taking it, what you noticed and whether you needed medical care.
• You can also report it directly to the FDA (MedWatch): 1-800-FDA-1088 or https://www.fda.gov/safety/medwatch.
• What we do with your report. We record it and keep the record for 6 years. If it is a serious adverse event, we send it to the FDA within 15 business days, as federal law requires (21 USC 379aa-1). • You can report without leaving your details. If you tick «You may contact me», we keep your email or phone and may write to you for details. If you do not tick it, the report is anonymous: we keep no contact details and do not link it to your account, and we record it all the same.
